Once we have entered you into our system we will send you an email with a link to your patient portal. The email will look similar to the one above. Your username will be your email address. Click on Access Portal.

Next you will need to verify your identity by entering in your date of birth and your last name. Click on Verify Information. Once you are verified, you will be prompted to create a password.

Now that you have your username (email address) and your password, you can now log into your patient portal and complete the forms. Click the button below to login.

Once you are logged in, click on the My Health icon in the upper left corner.
Please note: If you are using your cell phone to access the patient portal, you will need to click on the three lines in the upper left hand corner in order to find the My Health icon.

Fill out each section on the left hand side, starting with Contact Info and ending with Family History.
Please note: You will not be able to enter any information in the Insurance and Pharmacy Section.

Once you are done with Family History your forms are now completed! Please call or text our office at (808) 323-2608 to schedule your first appointment.
Remember: Your patient portal offers you access to your scheduled appointments, test results, and billing information.

Island Dermatology LLC offers dermatology visits by video conferencing. Real time or interactive teledermatology involves the practitioner and the patient interacting through a live video conference. You can connect with the practitioner on your smart phone, computer or tablet/IPad. Our office will help walk you through the steps to connect with the practitioner for your assigned appointment time.
